It would.
Making the two plugins talk to each other would significantly increase the
size of the plugin. However, since the plugins architechture is now open,
people have expanded it to include video, namely the new Excite web-top box.

Yeah obviously jamming the QT plug-in into the flash plug-in would not be a
good idea.

However, a much more elegant solution would be to allow video files to play
through swfs (in a similar way to shockwave: the video file still depends on
the user having the appropriate plug-in).

This will never happen to avoid diluting demand for Director, but I
seriously doubt the amount of code required would make the flash player much
larger.
